Introduction

Park Avenue Fertility (PAF) is an andrology laboratory and sperm bank specializing in technological services for reproductive medicine. PAF provides male factor testing for clients referred by their or their partner's physician as part of the couple's fertility work-up. PAF offers the following tests and services:

Semen Analysis with Strict Morphology
Performed to determine the characteristics of a semen specimen. It is recommended to abstain from an ejaculate for approximately 2-4 days prior to having a semen analysis. Analysis parameters include:

- Liquefaction Time
- Volume- Viscosity
- pH
- Sperm Cell Concentration
- Sperm Motility
- Sperm Forward Progression
- Round Cells
- Agglutination / Aggregation
- Extracellular Debris
- Morphology (Abnormal Cells)

Semen Cultures
Performed to assess the presence of infectious organisms in a semen specimen. Cultures available:

- General Bacteria
- Ureaplasma/ Mycoplasma
- Chlamydia

DNA Testing (Urine)
Performed to assess the presence of infectious organisms.   Tests available (performed on urine specimens):

- Ureaplasma/Mycoplasma Sp by PCR
- Chlamydia Trachomatis/Neisseria Gonorrhoea by NAA

Sperm Wash for Insemination
Preformed to wash a semen specimen to use for an intrauterine insemination.   Preparation methods available:

- Gradient Sperm Wash
- Basic Concentration Sperm Wash
- Swim-Up Sperm Wash

How do I go about making an appointment for testing?

In order to have testing performed at Park Avenue Fertility (PAF), you must obtain a written physician's authorization from your physician or your partner's physician. Authorization can be in the form of a prescription pad note, letter, or other written document and should have the following information:

- the physician's name, address, phone #, and fax # pre-printed on the authorization
- client's name and date
- the physician's signature
- the name of the test required
- diagnosis code (optional)

You must call PAF to set up an appointment. All appointments must be scheduled in advance. In addition to   the physician's authorization, clients must fill out and submit PAF's Laboratory Registration - Andrology Services form.

Specimens can be:

- collected on-site at PAF in a private, clean, and comfortable environment.
- collected off-site (i.e. home/hospital room) and brought to PAF by the client or family member.

If a specimen is collected off-site, clients must fill out and sign Form #4 - Document of Off-Site Specimen Collection .Please call PAF for details on the procedure for producing a specimen off-site.

HOW DO I OBTAIN THE RESULTS OF MY TESTS?

A complete written report will be sent to the physician ordering your tests, and to any other physician as per your written request.   You may obtain the results from your physician.   According to New York State Department of Health regulations, PAF is unable to give written or verbal results directly to clients.

What are the fees for semen analysis and various fertility tests?

All fees are payable in full on the date of service. PAF does not accept insurance as payment for services rendered, but will furnish you with an itemized receipt.
